Changing the landscape in community healthcare

Foundation Clinic

In partnership with the UFH’s Center for Primary Care, Practice and Education Medical Fellows, UFCH is now able to offer clinical care for vulnerable adult patients in addition to our partner foster homes and orphanages. As part of their Primary Healthcare 3-year training program, Medical Fellows staff the Foundation Clinic on Monday and Friday afternoons and see patients for 60-90 minute visits. The Foundation Clinic focuses on Primary Healthcare and continuity of care for each patient even if the patient is referred to specialist care.

What Kind of Care

  • Physical examination
  • Gynecological
  • Specialist referral if recommended

Our Foundation Clinic process

  • UFCH Partner organization makes referral of care to UFCH
  • UFCH approves or denies care
  • Partner organization calls the Foundation Clinic for an appointment and informs UFCH
  • Patient attends first appointment in care of the referring organization, future visits may be attended without referring organization with permission from UFCH
  • Follow up appointments are made in same fashion as the first, all appointments must be pre-approved by UFCH
